ESP LTD active pickups. Includes everything pictured. Note the deals repair on the neck pickup, appears to have been soldered to keep the screw tag in tact, its how it was when I purchased them, it has worked without issue for me. Soldering was not done very cleanly, again I purchased them that way, but they still worked great when I plugged them in.
